Forgot Password? | Change Password MyHealth Access Network User License Agreement I understand that as an authorized user of MyHealth Access Network, I am subject to HIPAA, HITECH, and potentially other laws regarding access to individually identifiable information and protected health information (PHI) submitted to MyHealth Access Network by participating providers. I or my employer has signed an agreement that imposes certain restrictions on my use and disclosure of PHI from I may access through MyHealth Access Network. By agreeing to this statement, I reaffirm that I am trained in the proper use of such information and understand my limitations under that agreement. As an Authorized User of MyHealth Access Network, I acknowledge and agree as follows: I am an Authorized User of MyHealth Access Network. I have been granted a distinct logon ID and I have a confidential password. I understand I will have to change my password from time to time. When creating my password, I will do my best to use passwords that are not easy to guess, will safeguard my password and will not allow others to use it. I will use MyHealth Access Network to access information relating only to individuals with whom I have a health care treatment, payment or operations relationship. I will not disclose any protected health information from MyHealth Access Network to any third party except for the purpose of fulfilling my duties based on my relationship with the patient insofar as my relationship with the patient justifies are for treatment, payment or operations, or to comply with law. I will undertake a reasonable and professional degree of care to protect the confidential nature of any protected health and proprietary information obtained from MyHealth Access Network. I have received training on HIPAA and HITECH guidelines and have access to my employer’s HIPAA policies and procedures, as well as MyHealth Access Network’s Policies at http://myhealthaccess.net/policies, and agree to comply fully therewith, as they apply to me. I will notify my employer, or MyHealth Access Network, as appropriate, immediately of any violations of these policies of which I become aware. I will comply with all applicable federal, state and local laws, rules and regulations with regard to access, use and disclosure of PHI. I acknowledge that any violation of this User License Agreement may result in modification, suspension or revocation of my access to MyHealth Access Network, and/or disciplinary measures by my employer, up to and including termination of access or employment, and applicable penalties under federal, state and local laws. I acknowledge that every lookup of patients I make is logged and discoverable, and that I may be required to justify access to any patient’s record I decide to view.
